The SUNY Empire Health and Wellness Initiative has brought back the free flu shot vouchers for all students. We’ve contracted with Integrated Health 21 to bring you free flu shot vouchers that you can use at nearby chain pharmacies. This year, we have expanded access to three national pharmacies:  

 

 

 

 

To obtain your free voucher:    

• Log on to the My.ESC Health and Wellness Page.  
• Find the quick link for ‘Flu Shot Voucher’ at the top of the screen.  
• Once on the Integrated Health 21 registration website, fill out the registration form with  
• Your first and last name  
• Your date of birth  
• The company code = SUNY  

  

Clicking ‘continue’ will guide you through the rest of the registration process, which includes selecting the pharmacy where you want to receive your flu shot. You may print out the voucher or show it on your phone at the pharmacy. If you have questions or trouble getting your voucher, please email basicneeds@esc.edu.
